COVID-19 UPDATE

HOW TO PREPARE BEFORE YOUR VISIT



Before your scheduled visit, we ask that you review the following:

PHARMACY UPDATE eff 1/1/2025 for Independent Health Members:

Note the change for Independent Health members who use CVS Pharmacies.  As of 1/1/2025

CVS will no longer participate with Independent Health Policies*. (Click Here to view letter) Therefore, a new pharmacy must be chosen.  Please have the new Pharmacy Name and Address ready upon checking in at your appointment. 

*Independent Health Medisource is the ONLY exception to this change


WELL SIDE:

If you or Your Child(ren) have had any of these SYMPTOMS in the last 3 days…

 *ANY COUGH OR RUNNY/STUFFY NOSE WITH ANY POSSIBLE FEVER*

*****Then Please put on MASKS and step over to the SICK WAITING ROOM.*****


(If you are not comfortable going to the Sick Waiting Room, then call 716-893-7337 from this hallway!)

SICK SIDE:

If you or your child(ren) have had any of the following symptoms in the last 3 days,  *ANY COUGH OR RUNNY/STUFFY NOSE WITH ANY POSSIBLE FEVER*  then please wear masks until the provider (doctor or nurse practitioner) makes a final determination whether it is recommended for this visit.


We do not do testing for COVID at our office; so we may ask parents to perform home tests, based on the child's symptoms & vaccine status, before performing an exam at the office.

Please arrive 10 minutes prior to your scheduled appointment.  Late appointments may be asked to reschedule.

NYS IMMUNIZATION REQUIREMENTS

Automated reminder texts/calls are still going out, please respond appropriately to avoid another call to you, from our office, to confirm this appointment. Appointments that are “missed” or are cancelled with less than 12 hr notice, will result in a $35 fee.


A Confidential questionnaire completed by the teen/adolescent is private between the patient and the provider. This is a measure of the "Best Practice" for Pediatric Practices.  This occasionally results in a "patient balance due" from your insurance carrier.


“We are ACTIVELY open” for all visits, but please try to come with minimal visitors/accompanying people.



Whenever possible, we recommend that you bring only the child(ren) who are scheduled to be seen in the office.



We have an excellent track record for prevention of disease spread due to separate entrances, waiting rooms, and exam rooms for our Healthy vs. Sick Patients.

Telephone Med Calls – Sick triage nurses and Providers may occasionally practice “phone medicine”, when symptoms of cough, fever, sore throat are present, or when portal pictures can be effectively sent. Please see the Patient Portal Info tab if there are portal questions.
